Dental Insurance

Three levels available:

  • Level 1: Routine Dental Services including: diagnostic, preventive, minor, restorative, endodontic, periodontic, and some oral surgery
  • Level 2: Major Restorative Services including prosthodontics and major restoration (crowns and bridges)
  • Level 3: Orthodontics for dependent children to age 19

Six plan designs:

  • 100% Routine Services
  • 80% Routine Services
  • 100% Routine 50% Major Services
  • 80% Routine 50% Major Services
  • 100% Routine 50% Major Services 50% Orthodontics
  • 80% Routine 50% Major Services 50% Orthodontics

Combined maximums for Routine and Major Services: $1,500 per year, per person.

Orthodontic maximum: $1,500 per child per lifetime.
